Responsibilities:
Provides crisis intervention for patients and families.Assesses social, emotional and economic stresses of high-risk patients and families, and assists with identification of appropriate resources.Provides psychosocial support to patients and families, as needed.Acts as a patient/family advocate.Coordinates appropriate reporting to legal agencies as needed with respect to abuse identification.Assists with the coordination of post-discharge needs and links patients/families to entitlements, community resources and other needed services.Coordinates discharge planning patient for patients transitioning to non-acute care settings; i.e., skilled nursing facility; hospice; personal care home, home healthcare, etc..Acts as facilitator and coordinator to the healthcare team with patients and families.Works as a team with designated unit's Utilization Review Case Manager to ensure highest level of customer service and care quality for all assigned patients

Master's degree in social work from an accredited university preferredLCSW or LMSWAt least 3 years in social work role. Supervisory skills required. Previous acute-care hospital and/or discharge planning experience preferred.
