Job Description

Purpose: ILP Program Assistant is responsible for coordination of services for all clients in the Community Residential Alternatives/ZH-ILP Program including CLS and Host Home services, ensuring that high quality services are provided to the supported individuals with measurable progress towards goals and the highest level of customer satisfaction.   


  • Collect field documentation as needed for office client files in accordance with regulatory standards (HCFR, CARF, Medicaid)
  • Manage and track all ILP/CRA residential finances, receipts, and records in coordination with Residential Managers. 
  • Maintain accurate and timely documentation on Therap. Maintain all electronic records.
  • Ensure that all ZH-ILP Residential Managers (RM), CLS staff and Host Home providers and staff are maintaining client records per HCFR, CARF and Medicaid standards.
  • Intake new incoming calls and participate in new client application review meetings. Coordinate new clients’ transition into ZH-ILP, including CLS and Host Homes.
  • Participate in Direct Support Professional (DSP) and RM interview process as scheduled by the Human Resources.
  • Coordinate staff training to work with clients and ensure completion of an orientation checklist.
  • Develop Individual Service Plans (ISP’s) for each client in the program in collaboration with the client, family, support staff, and state/regional agencies at least once yearly.
  • Increase the function of direct support by performing home visits at least monthly during rendering of support to provide on the job training, supervision and feedback.
  • Work with staff to implement ISP and ensure that goals are being worked on/met regularly and are documented correctly.
  • Ensure that DSPs are trained on the client’s Behavior Support Plan. Ensure that DSPs are implementing the behavior support plan as written and trained, and are completing behavior tracking thoroughly and accurately.
  • Document all staff training for personnel records in collaboration with the Staffing Coordinator.
  • Collaborate with Program Manager to participate in the hiring, evaluation, disciplining of staff.
  • Maintain consistent and regular communication with clients and families to help ensure that all aspects of client needs, including those described in the ISP, are being addressed.
  • Work with clients and family/guardian to identify and obtain community resources as needed.
  • Ensure that the Service Plan of Care is consistent with the level of care needed.
  • Perform visits in the client homes (at least one face-to-face a month), checking to see that the home environment is safe and appropriate, clients appear to be safe and in good health, and client needs are being met (staffing, roommates, work, health, socialization, etc.)
  • Carry 24/7 emergency phone on a rotating basis and respond to client/family emergencies appropriately and in a timely manner.
  • Ensure that emergency fact sheets are complete, accurate & available in emergency book and Therap.
  • Maintain awareness of client medical needs and ensure regular communication with the RN.  
  • Collaborate with other programs and agencies as needed.
  • Participate in all I/DDS area Staff Meetings and trainings as required

Minimum Requirements:

  • Ability to organize, problem-solve creatively, and multitask.
  • Ability to empathize and a willingness to learn.
  • Ability to review and maintain accurate and timely documentation.
  • Excellent written and verbal communication skills.
  • Experience with Excel.
  • Must be PCA Certified or willing to become certified.
  • Must have a Georgia State Driver’s License and a privately insured vehicle.

Education and/or Experience:

  • Bachelor’s Degree in Social Services or related field and at least 2 years of experience with individuals with intellectual and/or developmental disabilities. One year of supervisory experience preferred.
