Pathways Center's developmentaldisabilities programs and services are currently based out of four counties inthe west, central Georgiaarea.  They are: Carroll, Heard, Coweta,Meriwether, and Troup counties.  Theindividual in this role will ensure a safe environment where individuals aretreated with dignity and respect.  The mainfocus of this role consists of supporting and facilitating opportunities forclients to participate in recreation, socialization, and vocational activitiesthat are determined based on their personal interests and skill sets, which isidentified through the person-centered planning process. 


TheDirect Support Professional provides individuals with developmentaldisabilities training and guidance in the everyday skills they need to helpachieve their highest level of functioning and to participate as fully aspossible in the surrounding community.  TheDirect Support Professional provides personal care, social support, andphysical assistance to individuals with disabilities in a wide range ofactivities of daily living.  This rolehelps individuals learn living skills such as cooking and shopping, and make itpossible to participate in employment and social activities in thecommunity.  Responsibilities for the DirectSupport Professional include but are not limited to the following:


?        Providesdirect personal care and supervision to developmentally and/or intellectuallydisabled residents in a facility based setting and/or community environment asneeded

?        Assistswith the development of an individual service/treatment plan for each assignedclient

?        Teachesbasic life skills in residential, day, and/or community based settings

?        Observesand monitors client's behavior and documents observations and incidents

?        Providestraining of basic personal and social skills, simple academics, or work skillsto clients

?        Arrangesfor medical care and provides support to ensure that medical care is obtained

?        Monitorsclient food intake, functional status, psychosocial status, wellnessactivities, etc.

?        Supportsindividuals in building relationships and social networks

?        Providestransportation for client(s) as needed for individual and/or group communityoutings

?        Actimmediately on any crisis, following protocol and basic CPR/first aid training

?        Counselsand advises clients on behavioral problems, daily decision-making, and the resolutionof minor problems

?        Participatesin Individual Service Plan meetings/Case Reviews consulting with the team toevaluate the client's needs and plan for continuing services

?        Performshousekeeping duties such as cleaning, washing clothes and dishes, and runningerrands

?        Involvesclients in the planning, shopping for, and preparation of nutritious mealstaking into consideration any dietary needs/requirements

?        Documentsclient progress and services provided on a daily basis, reporting any changesin client condition to all members of the client's direct care treatment team

High school diploma or GED AND any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities. Note: Some positions may require a valid driver's license.


License as a Certified Nursing Assistant (CNA)
