The Burke Center - Griffin, Ga.

Case manager will facilitatesubstance abuse (SA) psych education groups, group's skills/ training andsubmit corresponding notes within the appropriate time frame. Case manager willprovide a broad range of social services to patients/ clients/ consumers andfamilies. Case manager will assist in inpatient/ client/ consumer intakes. Mayprovide supportive counseling to consumers and families and or serve as aliaison for social services. Case manager will complete staffing forms on eachindividual as needed to report progress of individuals. Case manager willensure charts/ documentation is compliant and within guidelines of CARF, DBHDDand DCH. Case manager will provide oversite for all individuals within the SAinterview to residential program.


NOTE: McIntosh Trail Community Service Boardconducts background checks on all final candidates. Hiring of applicants iscontingent upon satisfactory results of:

Fingerprint BasedBackground Check of the Georgia and Federal Criminal History Record 
Database
Drug Screen
Motor Vehicle Report (MVR) for seven years

TB testing will be required upon appointment. If you have previously testedpositive for TB, you will be required to bring a copy of your chest x-ray onyour first day of orientation.

Qualifications

Bachelor's degree in a social services related field from an accredited college or university OR High school diploma or GED AND Two years of experience in a social services related position OR One year of experience at the lower level Social Svcs Tech 1 (SST010) or position equivalent.
