Residential Care Provider

Provideresidential support services for Horizon Respite clients. This includes but itnot limited to the following: Meet face-to-face with the clients daily to makea high-level assessment of current condition; inquire about client needs; andreview client's itinerary for the day. Provide skills training to clients aminimum of 1-2x/week. Provide groceries/household items and/or transportationto the grocery store/food pantry. Provide transportation to doctorappointments, job interviews, and government offices as needed. Providetransportation to View Point services as needed. Assist clients in learning tonavigate public transportation. Monitor client medication compliance daily andassist with securing refills as needed. Perform unit inspections forcleanliness and safety, assist with daily cleaning as needed, and prepare unitsfor new clients within 24 hours when beds become available. Complete emergencydrills for each unit on a quarterly basis. Enter paraprofessional notes within24 hours that reflect the services and specific skill training that wasoffered. Applicant should be able to lift groceries and items up to 15 pounds.
High school diploma or GED AND any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities. Note: Some positions may require a valid driver's license.
